Click Here for Visitor Consent Form.

Visiting an incarcerated person is a privilege, not a right. In order to limit the spread of COVID-19, the DOC will screen all visitors for COVID-19. The DOC will share the identities of all visitors with the Division of Public Health in order to trace visitors who show no signs of illness at the time of visit, but subsequently test positive for COVID-19. By visiting an incarcerated person, a visitor consents to screening for COVID-19 and consents to sharing the fact of visitation with the Division of Public Health.
